Can You Afford Your College Choices?

This form is used to authorize the counseling office to provide information to a college, university, technical school, or other post-secondary program AFTER you have completed/submitted the application to the college. This form must be completed and delivered to the Counseling Office. Please note that this is not necessary for the common app or Penn State University.

College Application Request

This worksheet helps you to compare technical schools, colleges, and universities in a methodical way, taking into account a significant number of school characteristics. This provides a visual organizer to help in the very difficult process of narrowing your choices down.

College Comparison Chart

College visits can be overwhelming. By the time you drive or even fly a great distance, locate parking, make your way to the registration area, and start your tour, you sometimes can use a nap! At that moment, thinking of all of the questions you want and need to ask can be difficult. This form helps to stay on track and ask the necessary questions.

Making the Most of Your College Visit

This form is to be completed and submitted for graduating seniors. It assists in everything from scholarship consideration and recommendations to press releases. Please note that you will be prompted to make a copy of the Google Doc (and thus must be signed in to Google). This will allow you to edit the form. You then should export it as PDF and email it to the counseling office or those writing recommendations.

Student Activities Form

Especially as a junior and senior, you will have a need to send your transcript to organizations or schools. The Counseling Office accepts transcript requests only electronically. Simply click the link below (while logged into your npsd.org account) to complete the Google Form.

Transcript Request Form

Students between ages 14-17 require a work permit, commonly referred to as "working papers," to be employed in the Commonwealth of Pennsylvania. Pennsylvania's established procedure requires the school in which a student is enrolled to complete the permit. To apply for a work permit, complete the steps below.

2. Scan (or photograph) and email the completed application, along with a scan or photograph of your birth certificate to the counseling department secretary (found at the bottom of the page below).
